PERFORMANCE WORK STATEMENT (PWS) Vehicle- Mounted Aerial Lift Platform (Bucket Truck) Repair and Certification 140th Wing, Colorado Air National Guard 1.0 GENERAL 1.1 Scope: This Performance Work Statement (PWS) outlines the requirements for a contractor to provide all necessary personnel, supervision, labor, parts, materials, tools, and equipment to perform comprehensive repairs on one (1) government - owned vehicle - mounted aerial lift platform (bucket truck). The objective is to repair all identified deficiencies and certify the equipment in accordance with manufacturer specifications and industry standards, rendering it safe for operational use. 1.2 Background: The 140th Logistics Readiness Squadron (LRS) manages a bucket truck that is a critical asset for base support operations. During its most recent mandated annual inspection, the equipment failed to meet safety and operational standards. Multiple critical and safety - related discrepancies were identified, requiring immediate corrective action. The 140th LRS Vehicle Management flight is not certified to conduct these specialized repairs. 1.3 Applicable Documents: All work performed shall be in compliance with the latest version of the following standard: * ANSI/SAIA A92.2: Vehicle- Mounted Elevating and Rotating Aerial Devices 2.0 PERFORMANCE REQUIREMENTS The contractor shall perform the following tasks to restore the aerial lift platform to a Fully Mission Capable (FMC) and ANSI- compliant condition: 2.1 General Repairs: The contractor shall furnish all parts and labor to complete the following: * Task 2.1.1: Replace damaged ISO- GRIP controller cover screws. * Task 2.1.2: Replace one to ten (1- 10) damaged or missing safety and instructional placards to meet manufacturer and ANSI standards. * Task 2.1.3: Replace the damaged platform rotation pin assembly. This shall include all time required to drill out the damaged pin to facilitate installation of the new assembly. * Task 2.1.4: Account for time required to drill out damaged nylon screws on the ISO - GRIP controller cover. * Task 2.1.5: Replace worn anti- slip tape on the platform entry step. * Task 2.1.6:Repair and replace the leaking lower control valve, identified as a Class 1 hydraulic leak. -- 1 of 2 -- 2.2 Platform Tilt System Repair: The contractor shall troubleshoot and repair the inoperable platform tilt functions. * Task 2.2.1: The scope of this task is open for cause and correction but shall include, at a minimum: adjusting and lubricating the handle, adjusting the proximity switch for manual leveling at the turntable, and lubricating the chain mechanism. The final repair must restore full, safe functionality to the platform tilt and leveling system. 2.3 Logistics and Fees: The contractor shall be responsible for the following: * Task 2.3.1: All freight and transportation costs associated with the repair actions. * Task 2.3.2: All necessary shop supplies and environmental disposal fees incurred during the repair process. 3.0 TESTING AND CERTIFICATION 3.1 Operational Testing: Upon completion of all repairs listed in Section 2.0, the contractor shall conduct a full operational test of the unit. This test shall cycle the aerial lift through all its functions to verify that all systems are operating correctly and safely, per manufacturer and ANSI A92.2 standards. 3.2 Final Certification: The contractor shall provide documentation certifying that the aerial lift platform is compliant with all applicable ANSI A92.2 standards and is safe for government use. 4.0 DELIVERABLES The contractor shall provide the following upon completion of services: * One (1) fully repaired and functional vehicle - mounted aerial lift platform. * A detailed invoice listing all parts and labor provided. * A copy of the final, signed ANSI A92.2 inspection and certification document. -- 2 of 2 --
