PERFORMANCE WORK STATEMENT (PWS) Page 1 of 32 Termite Pest Control/Elimination Service for Subterranean and Dry- Wood Termites, U.S. Army Garrison (USAG) Hawaii Directorate of Public Works (DPW) Island of Oahu, Hawaii 26 May 2026 v12 Part 1 General Information 1. GENERAL. This is a non-personal service contract under which the personnel rendering the services are not subject, either by the contract’s terms or by the manner of its administration, to the supervision and control usually prevailing in relationships between the Government and its employees. The United States (U.S.) Government shall not exercise supervision or control over the Contractor’s employees performing the tasks herein. Such contract employees shall be accountable solely to the Contractor who, in turn shall be responsible to the U.S. Government. The contractor shall provide Fumigation/Elimination of Subterranean and Dry-Wood Termites for various buildings at Army Installations on the Island of Oahu. 1.1 Description of Services/Introduction. The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to perform Subterranean and Dry-Wood Termite Pest Control/Elimination Service as defined in this PWS except for those items specified as government furnished property (GFP) and government furnished services (GFS). The contractor shall perform to the standards specified in this contract. 1.2 Background. To maintain and sustain the historical integrity of our facilities and buildings by preventing wood-to-soil contact, managing moisture, maintaining ventilation, and considering professional termite control. 1.3 Objectives. The objective of this contract is to ensure all work accomplished in the performance of this PWS meets all applicable Federal, State, and Local Laws, regulations, and directives. 1.4 Scope. The Contractor shall provide non-personal services for termite fumigation/elimination of Subterranean and Dry-Wood termites to include termite pest surveillance, termite pest identification, termite breeding site removal, termite pesticide applications, and other required actions to control termite pests in and around buildings and facilities. The contractor shall accomplish unscheduled Termite Pest Control/Elimination Services for public health pests for real property facilities on Army Installations located on the Island of Oahu. Termite Pests are defined as Subterranean and Dry-Wood termites that adversely affect readiness and military operations, or damage real property, supplies, and equipment. 1.5 General Information. 1.5.1 Buildings and Structures. The Contractor shall perform Fumigation/Elimination of Subterranean and Dry-Wood termites for various buildings at Army installations on the Island of Oahu. -- 1 of 32 -- PERFORMANCE WORK STATEMENT (PWS) Page 2 of 32 1.5.2 Regulations. The Contractor shall provide all services and storage procedures in accordance with (IAW) the following. a. Federal, State, and Local Laws and Regulations b. Department of Defense Instruction (DoDI) 4150.07, DoD Pest Management Program c. AR 200-1, Environmental Protection and Enhancement d. U.S. Army Garrison (USAG) Hawaii Integrated Pest Management Plan (IPMP) e. Federal Insecticide, Fungicide, and Rodenticide Act (FIFRA) f. Hawaii Department of Agriculture g. United States Environmental Protection Agency (USEPA) regulations 1.5.3 Army Policy. The Contractor shall follow Army policy that requires DD Form 1532, Pest Management Report and DD Form 1532-1, Pest Management Maintenance Record to be used to record and report all pest control operations, to include surveillance. 1.5.4 Motor Vehicle Operators. Contractor’s personnel, whose tasks involve operation of any vehicles, shall possess a valid driver’s license, certificates, and permits applicable for the type and class of vehicle being operated. 1.5.5 Environmental Protection. All environmental protection issues shall be coordinated with the COR/ACOR for obtaining recommendations and guidance from the Entomologist or the Directorate of Public Works Environmental Branch and approved by the Contracting Officer (KO). 1.5.6 Licenses and Certifications. The Contractor shall provide its business license and pest management specialist’s names, certification numbers (DoD or State), certification categories, and expiration dates within thirty (30) days of start of the contract, and annually by end of the performance period. The Contractor shall ensure that the licenses/certificates and training of Contractor personnel are kept current. Copies of these licenses/certifications shall be provided to the KO and COR upon request. 1.5.6.1 The Contractor shall ensure all Contractor’s employees and associated subcontractors are certified or licensed in the specific areas required by the State of Hawaii. The Contractor shall ensure employees are certified or licensed in the appropriate Environmental Protection Agency (EPA) areas required. The Contractor shall ensure all required documentation of certification or licensure for Contractor personnel are filed with the COR/ACOR within twenty-four (24) hours prior to employees commencing performance. 1.6 Quality Control Plan (QCP). The Contractor shall develop, implement, and maintain a QCP and program to ensure all work described in this contract is performed at or above the standard defined in the Performance Requirements Summary (PRS). The QCP is developed by the contractor for its internal use to ensure that it performs and delivers high-quality service. The contractor’s QCP is how the contractor ensures all work complies with the requirements of the contract. The QCP identifies and corrects potential and actual problem areas throughout the entire scope of the contract. 1.6.1 The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services.
