ATTACHMENT 2PERFORMANCE WORK STATEMENTWESTERN OREGON SERVICE UNIT MEDICAL WASTE DISPOSAL Solicitation No. 75H71326Q00036 Chemawa Indian Health Center, 3750 Chemawa Road NE, Salem, Oregon 97305 1.0 Title Western Oregon Service Unit Medical Waste Disposal 2.0 Background The Chemawa Indian Health Center requires routine biohazardous/infectious/regulated medical waste collection, transportation, treatment, and disposal services to ensure the safety of patients, staff, and visitors. Proper disposal of regulated medical waste is necessary to maintain compliance with applicable federal, state, and local requirements and to support accreditation standards, including AAAHC certification requirements. This service is essential to maintain a safe clinical environment and prevent exposure to potentially infectious materials. 3.0 Scope The Contractor shall provide all labor, management, supervision, materials, equipment, containers, liners, secure exterior storage cabinet, transportation, treatment/disposal, manifests, and documentation necessary to collect and dispose of regulated medical waste generated by the Chemawa Indian Health Center. The work is a non-personal commercial service. The Contractor shall perform in accordance with all applicable federal, state, and local laws, ordinances, permits, licenses, franchises, and regulatory requirements. 4.0 Place of Performance Service location: Chemawa Indian Health Center, 3750 Chemawa Road NE, Salem, Oregon 97305. Government acceptance will occur at the Western Oregon Service Unit. 5.0 Period of Performance Base Period: The base period shall begin within 30 calendar days after award and continue for 12 months. The actual start date and end date will be stated in the resulting award. Option Period 1: 12 months following the base period, if exercised. Option Period 2: 12 months following Option Period 1, if exercised. Option Period 3: 12 months following Option Period 2, if exercised. Option Period 4: 12 months following Option Period 3, if exercised. 6.0 Estimated Utilization Historical/estimated utilization is provided for planning and pricing only and is not a guaranteed quantity. The facility is currently billed for 35-gallon regulated medical waste boxes, collected once per month. The exterior storage cabinet has an estimated maximum capacity of eight (8) 35-gallon boxes. The Government estimates up to eight (8) 35-gallon boxes per monthly pickup and one (1) additional high-volume pickup per contract year. Estimated annual utilization: up to 104 35-gallon boxes per contract year, calculated as up to eight (8) boxes per pickup multiplied by thirteen (13) pickups per year. The annual fixed price shall include the secure exterior storage cabinet, routine monthly pickups, one additional pickup per contract year, transportation, treatment/disposal, manifests, and disposal documentation. 7.0 Performance Requirements Task Requirement Performance Standard Deliverable/Documentation Acceptance Method 7.1 Containers and Liners Provide contractor-standard regulated medical waste boxes/containers and compatible liners. Containers shall be rigid or equivalent puncture-resistant material suitable for transport, liquid-impermeable when lined, capable of secure closure, and marked with the universal biohazard symbol and required warning language. Containers are available before waste accumulation exceeds storage capacity; containers are intact, clean, correctly labeled, and suitable for regulated medical waste. Containers, liners, labels, and any instructions needed for proper closure. COR or designated facility staff inspection. 7.2 Secure Exterior Storage Cabinet Provide and maintain a lockable, weather-resistant exterior storage cabinet for temporary storage of full regulated medical waste containers before pickup. Cabinet remains lockable, secure from unauthorized access, weather-resistant, sanitary, clearly marked with biohazard signage, and sized to hold at least eight (8) 35-gallon boxes or contractor-standard equivalent. Cabinet installed/maintained and corrected if damaged or unsanitary. COR or designated facility staff inspection. 7.3 Routine Collection Collect regulated medical waste at least once per calendar month. Routine service shall include pickup of full containers, replacement of supplies as needed, transportation, treatment/disposal, and documentation. Monthly pickup completed as scheduled and without avoidable service interruption. Waste is not allowed to accumulate beyond cabinet capacity due to Contractor delay. Pickup record/manifest for each pickup. Review of pickup records and facility confirmation. 7.4 Additional Annual Pickup Provide one (1) additional high-volume pickup per contract year when requested. Pickup occurs within 72 hours after Government notification unless otherwise approved. Pickup record/manifest for additional pickup. Review of pickup records and COR confirmation. 7.5 Regulatory Compliance Collect, transport, treat, and dispose of regulated medical waste in accordance with all applicable federal, state, and local requirements, including OSHA, DOT, EPA, Oregon Health Authority, Oregon DEQ, and applicable City of Salem/Marion County requirements. No regulatory noncompliance attributable to Contractor performance. Contractor maintains all required permits, licenses, and legal authority to perform the work at the service address. Permits/licenses/authorizations upon request; required certifications, manifests, treatment/disposal records, and related documentation. Document review and regulatory compliance monitoring. 7.6 Manifests and Disposal Documentation Provide all required manifests, shipping papers, certificates, treatment/disposal records, and documentation required by regulation or requested by the Government. Documentation is complete, legible, accurate, and submitted with or before the applicable invoice unless otherwise approved. Monthly invoice package
