INVOICING -- 2 of 7 -- 2 STATEMENT OF WORK (SOW) 1. INTRODUCTION a. The primary mission of the Defense Logistics Agency (DLA) Disposition Services (DLA DS) is to dispose of Department of Defense (DoD) excess and surplus personal property generated by the military services. This is accomplished through reutilization to other DoD activities, transfer to other Federal agencies, donation to authorized state and local organizations, sale to the public, or ultimate disposal through abandonment or destruction. DLA DS is headquartered in Battle Creek, Michigan, with a variety of field sites worldwide. The purpose of this contract is to acquire new 4K Forklifts at DLA DS Sites in the Continental United States (CONUS). b. This is a Firm-Fixed Price (FFP), Indefinite Delivery Indefinite Quantity (IDIQ) Contract. The contract consists of a period of performance for one (1) 12-month base period and four (4) 12-month possible option periods. The total length shall not exceed five (5) years or 150% of the maximum estimated value of the entire contract. The Option Periods are not guaranteed. c. Upon receipt of a task order, the Contractor has 6 months to deliver the forklift. The Contracting Officer (KO) may extend the delivery period for extenuating circumstances. Ten (10) days prior to the end of the period of performance, a written request for extension must be received from the Contractor detailing the circumstances that delayed delivery. The written notifications will include the subject Task Order (TO) number, reason(s) for the delay with supporting substantiated actionable documentation, as applicable, and the estimated date and time for completed performance. d. New Equipment ONLY; no remanufactured or “gray market” items. All Forklifts will include all cables, batteries, chargers, and accessories necessary to make them operable able to be recharged and maintainable upon delivery. e. The forklift will have a DoD item unique identification (IUID) in compliance with DFARS 252.211-7003 (252.211-7003 Item Unique Identification and Valuation. | Acquisition.GOV). 2. CLIN 0001 4K ELECTRIC REACH TRUCK FORKLIFT SALIENT CHARACTERISTICS When CLIN 0001 is ordered, the Contractor shall purchase and deliver one (1) 4K Electric Reach Truck Forklift with the following salient characteristics: 1) Forklift Type: Reach Truck Forklift 2) 4000 lb. lifting capacity 3) Load Center 24 inches 4) Power: Electric 5) Tilt (forward/back): 3/4 degrees -- 3 of 7 -- 3 6) Overall Maximum Length 77 inches 7) Height to Top of Overhead Guard 95 inches Maximum 8) Minimum turning radius 75 inches 9) Reach stroke – 24 inches Minimum 10) Fork length – 42” (+/-1”) 11) Fire Extinguisher 12) Visible amber LED strobe light– continuously activated, overhead guard mounted 13) Audible reverse activated alarm) 15) Solid Rubber Tires (Solid polyurethane steer tires are also acceptable) 16) Forklift Charger (All items necessary to power/charge/recharge forklift. Electrical chargers, docking station, cables, plugs, batteries, voltage meters, etc.) 17) Maintenance Free batteries (Types of maintenance free batteries include Sealed Lead-Acid (SLA) Batteries, Gel Batteries, Lithium-ion (Li-ion) Batteries, Thin Plate Pure Lead (TPPL) Batteries) 18) The battery shall be rated to provide 8-hours of continuous use 19) An overhead guard shall be furnished. The overhead guard shall meet all of the requirements of ANSI/ITSDF B56.1 including falling object protective structure (FOPS) and inside clearances. 21) Lift Hight Minimum 13’ 3. CLIN 0002 4K ELECTRIC FORKLIFT STAND-UP WITH 48” TINES – SALIENT CHARACTERISTICS When CLIN 0002 is ordered, the contractor shall purchase and deliver one (1) 4K Electric Stand-Up forklift. This forklift must be a stand-up model and meet all the characteristics detailed in Section 2, except for reach capability, and the tine length, which shall be 48 inches. A tolerance of plus or minus one (1) inch is acceptable for all fork length measurements. 4. CLIN 0003 4K ELECTRIC FORKLIFT SIT-DOWN WITH 36” TINES – SALIENT CHARACTERISTICS When CLIN 0003 is ordered, the Contractor shall purchase and deliver one (1) 4K Electric Sit-Down forklift with the following salient characteristics: 1) 4,000-pound lifting capacity 2) Fork length – 36” (+/-1”) 3) Open Cab 4) 3-Stage high visibility mast 5) Hydraulic fork positioner 6) Hydraulic side shifter 7) Visible amber LED strobe light – continuously activated, overhead guard mounted 8) Audible reverse activated 82-102 dB(A) self-adjusting alarm 9) LED Work Lights (Two front, one rear work light and two brake/tail backup lights) 10) Side Mirrors -- 4 of 7 -- 4 11) Full suspension seat 12) Solid rubber tires 13) Fire Extinguisher 14) Forklift Charger (All items necessary to power/charge/recharge forklift.
