Please confirm whether this is primarily a service-only maintenance contract or if the Government also expects the contractor to perform minor system upgrades, component replacement, and hardware refresh work under this award. See Section 1.2 of the Perfonnance WotK Statement (PWS) (Repairs, Diagnostic Testing and System Programming are listed with Preventative Maintenance) 2. Please confirm whether the historical workload estimate of 20 hours per quarter per site is intended to represent the full expected recurring labor baseline for PM, service coordination, and telephone support. The20 Hours per quaffer referenced in Attachment 3 is the full expected suppotf, but should be noted that it isjust an estimated quantity. 3. Please confirm required response times for: Standard service requests - Routine quatferty services Urgent outages - See Section 5.1.2. of the PWS for repairs. Shall be completed within five (5) business days. Mission-critical/ executive conference failures - See Section 5.1. 1. of the PWS for Service calls. The contractor shall respond to the NHJAX or BHC Management lnfonnation Depatfment (MID) point of contact (POC) by telephone call within two hours of receipt 4. Please confirm whether there is a required on-site response SLA by severity level. There is no SLA by severity level stated in the PWS. <span new="" roman",serif=""> 5. Prevailing wage? All duties will be incorporated into one rate per period of perfonnance(Preventative Maintenance, Repairs, Diagnostic Testing and System Programming) 6. Please confirm site access requirements for Naval Hospital Jacksonville, Kings Bay, and Mayport, including base access, badging, escort requirements, background checks, and lead times for site clearance See Section 1. 12.2. of the PWS for Base Access. NAS JAX uses DB/Os for continued base access and is renewed annually. Once all documents are received processing takes approximately 2 weeks. Branch HeaHh Clinics require base escotf and should be coordinated through the COR for specific technical POCs located at those sites. 7. Please confirm whether work may be performed during normal business hours only, or if after-hours service windows are preferred for clinical and conference spaces. The hours of operation won't vary from Section 1. 7. 3. of the PWS. 8. Please confirm whether parking, unloading, and tool staging areas will be made available at each site. Dedicated parking, unloading, and tool staging areas will be available for each site. 9. Please confirm whether all listed equipment is currently installed, operational, and in service. All equipment is currently installed but at the time of award ff any equipment has been replaced it will be identified in the post award inventory. 10. Please confirm whether any listed equipment is currently offline, obsolete, unsupported, or scheduled for replacement All equipment is currently online and within warranty specifications. Upgrades have been made within the past 24months and replacement is not required at this time. 11.
