Page 1 of 6 Page 1 of 6 Statement of Work (DRAFT)Preventive Maintenance and Repair of Steris Sterilizers VA Sierra Nevada Health Care System Section 1: General Information General: This is a non-personnel services contract to provide preventative maintenance and repair services for Steris Sterilizers and Washers at the VA Sierra Nevada Health Care System (VASNHCS). The Government shall not exercise any supervision or control over the contract service providers performing the services herein. Such contract service providers shall be accountable solely to the Contractor who, in turn is responsible to the Government. Background: VASNHCS owns a range of Steris sterilization and reprocessing equipment covered under this agreement, including multiple AMSCO 7053L steam sterilizers, an AMSCO 400 steam sterilizer, V-PRO Max low-temperature sterilizers, IW Pro SI instrument washers, Reliance large cart and utensil washers, Advantage Plus automated endoscope reprocessors, and endoscope drying cabinets. The agreement also covers associated system components such as loading and transfer car assemblies, motorized and non- motorized conveyor modules, and transfer carts that support sterilizer and washer operations. These devices require scheduled preventive maintenance at manufacturer- specified intervals and responsive repair services to ensure safe and uninterrupted SPS workflows Period of Performance: Base Year: October 1st, 2026- September 30th 2027 Location: VA Sierra Nevada Health Care System 975 Kirman Avenue Reno, Nevada 89502 Hours of Operation: The services for infection prevention equipment will be performed Monday through Friday, 8:00 A.M. to 5:00 P.M., local time, excluding federal holidays (“Normal Working Hours”). -- 1 of 6 -- Page 2 of 6 Page 2 of 6 Section 2: Definitions Definitions: Contractor. A supplier or vendor awarded a contract to provide specific supplies or service to the government. The term used in this contract refers to the prime. Subcontractor. One that enters a contract with a prime contractor. The Government does not have a contract with the subcontractor. Work Day. The number of hours per day the Contractor provides services in accordance with the contract. Work Week. Monday through Friday, unless specified otherwise. Section 3: Government Furnished Property, Equipment, and Services None Section 4: Contractor Furnished Items and Services The Contractor shall provide all equipment, supplies, management, supervision, personnel, and transportation necessary to assure that all services are in accordance with the contract and all applicable laws and regulations. The contractor shall ensure all work meets performance standards specified in this SOW and referenced documents. Section 5: Specific Tasks Overall Scope The preventive maintenance of the various sterilizers, washers, and disinfectors will be performed according to the recommended OEM frequency and scheduled in coordination with Biomedical Engineering. The preventive maintenance of the sterilizers and washers will be performed according to the manufacturer’s recommended procedure as stated in the manufacturer’s service manuals. This also includes preventive maintenance and repair of the loading/transfer car assemblies, transfer carts, and steam filters that are used with the sterilizers, as well as an annual chamber cleaning. -- 2 of 6 -- Page 3 of 6 Page 3 of 6 Service Reports After each repair and scheduled maintenance visit, the Contractor shall provide a written service report indicating the date of service, the model, serial number and location of equipment serviced, the name of the service representative, and the services performed and parts replaced. The reports shall be delivered to the Biomedical Engineering Office located at VA Sierra Nevada Health Care System 975 Kirman Avenue Reno, Nevada 89502 Or reports may be delivered electronically to the COR or designated representative. Preventive Maintenance: As noted under the Preventive Maintenance Schedule, the inspections will be coordinated with Sterile Processing & Supply (SPS) and Biomedical Engineering. Maintenance will comply with all applicable regulations and manufacturers specifications, and it will include, but is not limited to the following: Ensuring that the equipment is operating within its specifications. Calibrating and cleaning. Replacing any worn or defective parts. Installing and maintaining all software updates/upgrades. Preventive Maintenance Schedule: Preventive maintenance tasks for all sterilizers and washers shall adhere to Biomedical Engineering’s preventive maintenance schedule which will be based on the OEM frequency. Parts: The Contractor shall furnish and replace all necessary parts as required to return equipment to full manufacturers published specifications. All parts supplied shall be OEM (Steris) parts and compatible with existing equipment. The contractor shall use new parts only. Re-built or backward-engineered parts are not permitted. Service Calls Contractor personnel will schedule with SPS and the Biomedical Engineering Technician in charge of the sterilizing equipment prior to performance of services. -- 3 of 6 -- Page 4 of 6 Page 4 of 6 Documentation The Contractor shall provide individual service reports that reference services performed on the equipment. The following information must be provided in each service report: Date of service Summary of action taken Equipment details (description, equipment ID, serial number, etc.) Labor hours Parts used The service report must be approved and signed by the Contracting Officer’s Representative (COR) or designated representative. A copy of each report must be left with COR or designated representative at the time the services were completed. ID Badges, Parking, and Smoking Policy Badges: All Contractor personnel are required to wear identification (ID) badges when on VA property and surrounding grounds.
