Background A blood culture is a medical laboratory test used to detect bacteria or fungi in a person's blood. Under normal conditions, the blood does not contain microorganisms: their presence can indicate a bloodstream infection such as bacteremia or fungemia, which in severe cases may result in sepsis. By culturing the blood, microbes can be identified and tested for resistance to antimicrobial drugs, which allows clinicians to provide an effective treatment. 3. Period of Performance Base 9/30/2026 - 9/29/2027 Option Year 1 - 9/30/2027 - 9/29/2028 Option Year 2 - 9/30/2028 - 9/29/2029 Option Year 3 - 9/30/2029 - 9/29/2030 Option Year 4 - 9/30/2030 - 9/29/2031 4. Maintenance—Emergent and Preventative A technical assistance center must be available by telephone 24 hours per day, seven days per week, with a maximum callback response time of one hour. Onsite support to be available 7:00AM – 7:00PM local time Equipment repair response time must be no more than 24 hours; a technician must be onsite within 24 hours of initial request. Service agreement to cover 7 days per week during normal business hours. Preventative maintenance must be performed as frequently as published in manufacturer’s operator’s manual and within 2 weeks of the scheduled due date. Each notification for an emergency repair service call shall be treated as a separate and new service call. A malfunction incident report must be furnished to the Laboratory upon completion of each repair call. The report should contain at a minimum: Date and time notified Date and time of arrive Serial number, type and model number of equipment. Time spent on repair. Must define daily, weekly and monthly maintenance tasks and identify which tasks are user level and service level. The Contractor shall be able to provide not only emergency equipment repair but also preventative maintenance all on all primary and back-up instrumentation, and any incremental support equipment. If the Contractor provides remote service support, the contractor has estimated a VA national MOU/ISA between Contractor and VA prior to award. If the Contractor currently has a valid MOU/ISA, the Contractor shall include a copy of the MOU/ISA as part of their quote with the filed name “VA MOU/ISA”. Noncontract Charges: The contractor shall not perform any service that will result in additional charges without prior approval from the Contracting Officer. Payment: Payment will be made upon receipt of a properly prepared invoice which references the period for which payment is due, the contract number and/or assigned purchase order number. The invoice must include the correct payment address. Waste Removal: The contractor is responsible to coordinate with the VA POC to remove and properly dispose of all waste related to the services provide on-site Documentation Service reports will be provided prior to departure from the VA facility for all service activities including preventive maintenance, emergency repair, and safety inspections, and must contain the following information: Date(s) and time period of service. Complete description of equipment serviced including model number and serial number. Complete description of equipment services performed including upgrades, updates and software changes made. Complete list of all materials, parts, or assemblies installed or replaced including serial numbers of parts where applicable. A statement of the complaint that initiated the service call. Total standard labor hours, total overtime/premium hours, and total travel hours expended during the repair. Maintenance—Software Upgrades and Upgrades The Contractor shall provide upgrades and updates to both the equipment hardware and software in order to maintain integrity of the system and technology, at no additional charge to the Government. These shall be provided as they become commercially available and at the same time as they are being provided to commercial customers. This requirement only applies to “system upgrades” that enhance the current model of equipment. This does not refer to replacement of the original piece of equipment however it does refer to significant changes in hardware operational capacity. Any updates or upgrades to software shall meet all VA Information Security, Hardware, Software and Network requirements (VA 6500). Hours of Operation Normal tour-of-duty hours for VISN22 Pathology & Laboratory Medicine and services (PALMS) is 7:30 am to 5:00 pm, Monday through Friday, excluding Federal holidays. However, the service is operating 24/7 and as such there may be instances in which the vendor’s workday may be outside of this period. The vendor is required to coordinate these times with the designated Program Manager. In accordance with 5 U.S.C. 6103, Executive Order 11582 and Public Law 94-97
