All Individuals must accept personal responsibility for safety and seek opportunities for improving the safety of our work environment. Everyone working on Tinker AFB is challenged to find, fix (if able) and report all hazards and near-misses. It is our shared duty to take every reasonable step and precaution to prevent accidents and preserve the health/safety of each contractor and government employee. Section One 1.1 Scope: The Contractor shall provide all labor, tools, equipment, test equipment, material, parts, quality control, transportation and inspection manuals necessary to perform (2) Preventive Maintenance (PM) annually to include telephone support and unlimited repair visits at the Oklahoma City–Air Logistics Complex (OC-ALC) in accordance with (IAW) the terms and conditions outlined herein and conform to the manufacturer’s equipment specifications and conforming to this Performance Work Statement (PWS). The equipment is listed in Equipment List. 1.1.1 The Contractor shall be responsible for ensuring all services, parts and Original Equipment Manufacturer (OEM) components provided under this contract are in accordance with (IAW) the terms and conditions outlined herein, conform to the manufacturer’s equipment specifications and this PWS. 1.2 Requested Services: The Contractor shall provide a one-year base contract with 4 one-year options comprehensive PM support contract. The comprehensive PM support contract shall consist of the following: 1.2.1 Two (2) Preventive Maintenance (PM) visits per year on the JEOL JSM-IT300LV Scanning Electron Microscope (SEM) (SN MP1372001250125) • Each PM visit shall include the following:  One complete cleaning of the SEM (i.e. column)  One complete cleaning of the Vacuum Pumps  Contractor shall perform regular system calibrations and software updates 1.2.2 Contractor shall ensure that the JEOL JSM-IT300LV SEM (SN MP1372001250125) is fully functional during the contract’s period of performance. • Upon system failure or operational problems, contractor shall respond within 24 hours of the trouble call. • The contractor shall provide telephone support of the SEM operating systems during normal business hours for troubleshooting and performance issues. • Engineer labor and travel costs for in-person repairs shall be included at no additional cost during the PM contract period of performance. • Contractor shall perform repairs on all systems provided with the original purchase and installation of the SEM (i.e. pumps, chillers, computer hardware and software, monitors). 1.2.3 Repair Parts for covered equipment shall be included in the PM agreement at no additional cost. -- 2 of 17 -- Page 3 of 17 1.2.4 Covered equipment shall include the JEOL JSM-IT300LV Scanning Electron Microscope (SN MP1372001250125) listed in the Equipment List: Model Description 377791 JSM-IT300LV SCANNING ELECTRON MICROSCOPE SN: MP1372001250125 1.3 Preventive Maintenance (PM): The objective of PM is to protect the equipment capability and investment by removing causes of failure, performing routine maintenance and providing adjustment to compensate for normal wear before failure occurs. The Contractor shall perform all PM’s IAW the OEM specifications. The Contractor shall perform two (2) PM per year on each of the JEOL JSM-IT300LV Scanning Electron Microscope (SN MP1372001250125) listed in the Equipment List. The Contractor shall provide a copy of the PM schedule to the Government Point of Contact (GPOC) within ten (10) business days after contract award. The Contractor shall notify the GPOC seven (7) business days prior to each scheduled PM visit. 1.4 Over and Above (O&A): O&A is defined as work discovered during the course of performing the repair efforts that is within the scope of the contract and necessary in order to satisfactorily complete the contract. The Contractor shall complete O&A requirements In Accordance With (IAW) Defense Federal Acquisition Regulation Supplement (DFARS) 252.217-7028. PGI 217.77. 1.5 Malfunction and Notification Process: Upon occurrence of an equipment or system malfunction, the GPOC will contact the Contractor by telephone, email, or fax and provide information below. If the GPOC notifies the Contractor via telephone, the GPOC will provide a written copy of the request by email or fax. • the equipment identification number (ID#) • a brief description of the malfunction • location of equipment • name and telephone number of the Point of Contact (POC) 1.5.1 Upon notification of the system malfunction, the technician shall arrive on site at the equipment location within two (2) business days or if overseas five (5) business days. 1.6 Equipment Evaluations: The Contractor shall provide a written Estimate Repair Report to the GPOC by fax or email no later than (NLT) one (1) business day after the completion of the inspection. As a minimum, the information cited on the Estimate Repair Report shall include: • the equipment ID# • date of evaluation • description of findings and recommendations • required parts and part numbers (P/N) • cost of parts • number of parts needed • estimated labor hours required to complete the repair -- 3 of 17 -- Page 4 of 17 Any recommendations cited shall consider: • the priority of the requested service • available source for obtaining new parts • the availability of the new parts • shipping and delivery schedules of parts • other factors which may be peculiar to the specific repair requirement 1.7 Authorizations for Repair Actions: Only the Contracting Officer (CO) can authorize repair actions.
