PERFORMANCE WORK STATEMENT (PWS) FOR Repair, Concrete Basewide VDYD 262008 SCOTT AIR FORCE BASE, ILLINOIS 29 May 2026 1. REQUIREMENT: a. BACKGROUND: There are several locations on the grounds where concrete sidewalks are cracked to an extent requiring removal and replacement. In the Vehicle Refueling Building 5006, there are seven (7) trench drains that have damaged grates and cracked and spalling concrete surrounding them. Additionally, in front of the POL Building 5036, the concrete pad has damage from heaving resulting in a full depth crack across its width. At the Vehicle Refueling Building 5006, the vehicle ramp immediately north of thebuilding, has been pressed into the asphalt pavement from regular use bearing the weight of vehicles during maintenance work,. There are four 2 IN to 3 IN deep impressions where the 1FT X 1FT plates supporting the four stands bracing the level section of the ramp have pressed into the pavement. Lastly, north of Building 5010 an ADA Ramp is needed to transition between the parking lot to the adjacent sidewalk. b. OBJECTIVES: The project objectives are as follows: i. Correct existing cracked and failing concrete walkway paths by replacing damaged concrete panels. ii. Replace damaged trench drains with new approved trench drain systems connecting to the existing drain piping. iii. Replace concrete pad in front of Building 5036. iv. Place new concrete foundation for the vehicle ramp immediately north of the Vehicle Refueling Building 5006. v. ADA Ramp added to parking lot north of Building 5010. c. PROJECT LOCATION: All work will be within the 126th Air Refueling Wing (area). Specific buildings where work will take place under this scope are Buildings 5006, 5008, 5010 and 5036 at Scott AFB, Illinois. 2. PROJECT ADMINISTRATION: a. SITE VISIT: Contractor is responsible to verify all measurements and conditions pre-bid. The Government intends to only hold one official site visit. Requests for additional site access must go through the 126th Contracting Squadron (126 CONS) point of contact (POC) listed in the Request for Proposal (RFP) letter. Any additional site access will be -- 1 of 13 -- made available to all Contractors solicited for this project. b. MEETINGS i. Kick-Off Meeting/Project Schedule – Kick-off meeting to occur within one week of NTP. Overall project schedule with milestone markers and 2-week look-ahead to be completed at kick-off meeting. ii. Contractor shall coordinate with the 126th Contracting Squadron (126 CONS) on bi- weekly meetings once the construction phase begins. This meeting shall include the review of an updated 2-week look ahead schedule. Contractor shall provide meeting minutes within 24 hours of each meeting and allow the Government up to 24 hours to accept or request changes to the minutes. 3. PROJECT SCOPE: a. BASE BID CLIN 001: Sidewalk Repairs i. -- 2 of 13 -- ii. Contractor shall break and remove 350 SY total of concrete sidewalk. See highlighted images above and table of estimated areas of sidewalk to be replaced and repaired below. Sidewalk removal to include removal of existing base rock. iv. The contractor shall prepare the subgrade to ensure a stable and compacted base suitable for new concrete placement. v. Contractor shall place base rock on subgrade. Ensure there are no soft spots, frozen areas, or standing water. Contractor to place base rock so that the surface of the rock is level providing positive drainage away from building structures. Add and compact a minimum 4” aggregate base course (or match existing thickness if greater). vi. Contractor to install expansion joint material between existing concrete and newly placed concrete. vii. The contractor shall form, place, and finish new cast-in-place concrete sidewalk butting up to the existing sidewalk. The completed work shall meet the following requirements: (a) Concrete: All new concrete shall have a minimum thickness of 4 inches and a minimum 28-day compressive strength of 4,000 PSI. The surface shall have a non-slip broom finish. viii. Apply/spray concrete cure. ix. Site Restoration (1) Upon completion of the concrete work and after the required curing period, the contractor shall remove all forms and construction materials. The contractor shall restore all adjacent areas disturbed by construction activities to their original condition. This includes, but is not limited to, grading and seeding any damaged landscape areas. -- 3 of 13 -- b. (CLIN 002): Replace Trench Drains and Surrounding Cracked Concrete i. Conduct site inspection to verify site conditions. Contractor to coordinate all utility locates and dig permits. See the marked-up plan sheet A9.1 Floor Pattern and Equipment Plan in Exhibit A7. ii. Contractor to perform flow test from trench drain connections to downstream observation points including Oil and Gas or Paint separators to verify the discharge pipes function properly prior to start of construction. iii. Contractor shall sawcut at 1 ft offset from the perimeter of the trench drains. iv. Contractor will remove the existing 8” thick concrete pavement around the trench drains leaving the rebar in place v. Contractor will remove the trench drain walls and floors as well as the anchors and angle brackets for the existing trench drain grates. vi. Existing drain pipes and hardware/fittings from the trench drains shall be protected and repaired/replaced to maintain their function. vii. Contractor shall be responsible for disposal of construction debris. viii. Install new Neenah Foundry R-4996-A1 grated cover trench drains or approved equal. Ensure the drains are sloped for proper drainage to existing discharge pipe locations. ix. Place the concrete for the removed pavement. Concrete shall be 4000 psi ILDOT mix with a f…
