Page 1 of 5 Statement of Work STATEMENT OF WORK PROJECT OK 23-W92 CADDO COUNTY RURAL WATER DISTRICT # 3 WATER LINE EXTENSION CADDO COUNTY, OKLAHOMA INTRODUCTION SUMMARY OF WORK TO BE PERFORMED A. Construct a water line extension for Caddo County Rural Water District No. 3. 1. Furnish and install 10,940 feet of 2-inch DR 11 HDPE water main and 870 feet of 4-inch DR11 HDPE as casing. 2. Directional Bore 815 feet total including 2 road crossings, 1 driveway crossing, and 1 creek crossing. 3. 2 open cut gravel road crossings totaling 55 feet. 4. Case the water main or maintain a 5-foot horizontal and 2-foot vertical separation requirement from all gas lines. 5. Alternate bid items 12-15: Furnish and install 4 water main taps, water meters, and water service lines to homes. This includes 400 feet of 1-inch PE and 870 feet of 1.5-inch PE and appurtenances. 6. Furnish and install appurtenances including all gate valves, flush hydrants, air relief valves, tracer wire, locate stations, anode bays, thrust blocks, and fittings. 7. Complete pressure testing and provide results to the project engineer. 8. Satisfactorily complete disinfection and bacteriological testing of completed water main. Provide results to the project engineer. 2 consecutive negative tests 24hrs apart. Re-sampling of positive tests is to be performed at contractor expense. 9. Connect installed water line extension to Caddo County Rural Water District No. 3. LOCATION & CONTACT INFORMATION A. Location: Caddo County Rural Water District No.3 existing main at the corner of CS 2620 and CR 1170, installation will be north along CS 2620 to 15139 CS 2620 and east on CR 1160 from CS 2620 to 18131 CR 1160. Project Engineer EXISTING SYSTEM LOCATION A. The existing water main is located approximately 1 and ½ miles northeast of Binger in Caddo County, Oklahoma. The location of the proposed water line extension is shown in Figure 1 below. Figure 1. The location of the proposed water line extension. EXISTING WATER MAIN A. The proposed water line extension will be connected to the existing Caddo County Rural Water District No.3 3” water main which is located in the southeast corner of the intersection of County Street 2620 and County Road 1170. WORK TO BE COMPLETED GENERAL A. Contractor shall inform project engineer and Rural Water District at least 3 days prior to beginning construction. Will be provided at time of award -- 2 of 5 -- Stephenson – Taylor Water Line Extension Page 3 of 5 Statement of Work B. Failure to perform work on the date scheduled will require 24-hour notice for rescheduling the work. C. Leave all work exposed until inspected and approved. Work not inspected before it is covered will be re-exposed at the Contractor’s expense. D. All work is to follow attached specifications. E. All work is to adhere to applicable regulations and standards listed in the attached specifications. F. All work performed and material installed is to be covered by a one-year warranty. G. Any rock removed is to be transported and disposed of by the contractor. H. All work is subject to inspection by project officer or their representative. I. All work is to follow Occupational Safety and Health Administration and other specified regulations. J. Items that require approval from project officer shall be submitted for approval before beginning work. 1. Submittals can be sent via email, fax, or hard copy. 2. Project officer shall approve, deny, or request further information from contractor within 3 weeks of submittal receipt. K. Contractor shall provide project warranties of specified durations at project close out. L. Contractor shall connect to existing 3-inch PVC water main at the existing valve. M. Gravel driveways are to be crossed per plan set utilizing open cut method and/or boring and restored to original condition. N. Contractor shall install flushing valves at ends of water main extension. O. Contractor shall disinfect water main as indicated in specifications and collect bac-T samples until 2 consecutive negative samples taken at least 24 hours apart are achieved. 1. Re-sampling of positive tests is to be performed at contractor expense. INDIVIDUAL WATER METERS A. Rural Water District must be notified and on-site when tapping installed water main for water service line and installing meter boxes. QUALITY ASSURANCE A. Perform work under the direction of personnel licensed in the state including but not limited to Plumbing. B. Perform hydrostatic testing according to Section 33 05 05.3100. C. Perform disinfection of the installed water main according to Section 33 11 13.0001. WARRANTY -- 3 of 5 -- Stephenson – Taylor Water Line Extension Page 4 of 5 Statement of Work A. Provide a minimum one-year warranty for all materials and labor, covering defects in the materials or workmanship. WATER LINES (SIZES: 1-INCH THROUGH 4-INCH DIAMETER IPS PE PIPE) B.
