The Fort Duchesne PHS Indian Health Center is located in Fort Duchesne, Utah and primarily
serves the Ute Tribes of the Uintah and Ouray Reservation.

II. Objectives
The purpose of this requirement to convert the fitness area into an office workspace that why
we are requesting additional furniture to accommodate the departments needs and install the
new furnace at following address.

III. SCOPE OF WORK

IV. GENERAL CONDITIONS
A. The contractor shall provide service, labor, materials, and equipment to complete the
work.
B. Codes and Standards. The Contractor shall adhere to the following code requirements
for work performed, including testing and inspection:
1. Occupational Safety and Health Act – CFR1926
2. U.S. Environmental Protection Agency and Utah State Department of Health
C. Work Safety
1. All work will comply with Occupational Safety and Health Act – CFR1926.
2. Contractor shall supply all required worksite safety equipment.
3. Contractor shall be responsible for complete and strict compliance with all Fort
Duchesne PHS Indian Health Center facility policies, including the safety policy.
D. Inspection

1. The Government reserves the right to inspect all aspects of work performed.
2. Government will verify and approve all material and workmanship of the work
performed.
E. Final Payment
1. Partial payment will need to be requested with an invoice and progress report to the
Contacts listed in Section F.
2. Final Payment will not be authorized until all work is complete. Final Payment will
be with an invoice and progress report to the Contacts listed in Section F.
