SOW 3/31/25 Page 1 of 8 Statement of Work for JBVAMC Heat Exchanger Replacement Place of Performance: Jesse Brown VA Medical Center 820 S. Damen Ave Chicago IL 60612 Project Objective: The purpose of this project is to replace the (4) Steam to Hot Water Converters/Heat Exchangers within Damen and Ogden buildings. Current Conditions: The existing Heat Exchangers are old and unreliable. The Heat Exchangers are responsible for: maintaining and changing temperature in the Damen and Ogden buildings; and providing hot water to clinical needs and connected equipment. The Heat Exchanger units are old, breaking continuously, and not reliable; and the dependent equipment cannot fulfill their duties due to the Exchangers being incapable to provide hot water and to maintain consistent temperatures, thus reducing efficiency and potentially damaging the equipment long-term. T he staff rely equally on the Heat Exchangers to provide hot water, ensuring that the systems within Damen and Ogden that depend on hot water can continue to operate smoothly. JBVA Medical Center requests the immediate replacement of the Heat Exchanger units to continue providing the very critical services. Part 1: Project Scope of Work Overview: The contractor shall replace (4) existing Heat Exchangers. The removal & installation of the Heat Exchangers will include the disconnection and connection of electrical power. The Contractor shall provide all personnel, equipment, supplies, facilities, transportation, pay for their own parking while on site, tools, materials, and supervision required to perform the following: Part 2: Detailed Project Scope of Work: 1. Deliver all materials to the site during normal working hours. 2. Remove existing (4) Steam Converters/Heat Exchangers and accessories from piping systems. SOW Attachment 1. 3. Install (4) Steam-Hot Water Converters/Heat Exchangers a. BacNet capable b. Installed on existing concrete equipment pads 4. Reconnect all piping to Converters 5. Replace all steam traps and shut off valves 6. Reinsulate piping with appropriate mechanical insulation, if necessary 7. Test system for leaks 8. Electrical 9. Minimum 1 year warranty -- 1 of 8 -- SOW 3/31/25 Page 2 of 8 10. Perform daily clean-up of work areas. 11. Project Coordination. Part 3: Period of Performance. All work under this contract shall be fully completed within 90 days from notice to proceed. Part 4: Applicable Performance Standards A. Contractor shall ensure that the equipment functions in conformance with the latest published edition of NFPA-99, OSHA, Joint Commission, VA, UL, NEC, state and local regulations. The equipment covered under this contract shall be maintained in accordance with the manufacturer's standards/specifications. Preventive Maintenance inspection/repair procedures shall be in accordance with the manufacturer specifications. B. Service Technician Requirements. All service technicians performing work under this Contract shall be properly licensed, certified, or otherwise trained, in accordance with federal, state, and local requirements, to perform the work under this Contract. Any such licenses, certifications, or training shall be kept current throughout the period of performance of this Contract. The Contractor shall provide a copy of any such license or certificate the next Business Day, as defined below, upon request by the COR. Part 5: Jesse Brown VAMC Additional Requirements 1. Comply with all Jesse Brown VAMC site specific requirements and SOPs including and not limited to: Key Policy, Badging Policy, Rules of Behavior, and Smoking Policy. 2. Normal delivery hours are 7:30 am to 3:30 pm Monday through Friday. a. Normal work hours can be modified only by written approval of the COR. b. No delivery by the Vendor on Federal Holidays and no delivery on the weekend when the Federal Holiday is on the respective Monday or Friday. c. Work in stairs, interior / exterior public areas shall be between 6:00 pm through midnight concluding by 6:00 am on the following day. d. Any work including and not limited to: noise, vibration, dust, odors, core drilling, hammer drilling, saw cutting, equipment moving in public corridors, soil compaction shall performed between 6:00 pm through midnight concluding by 6 am on the following day. e. Crane lifts, any work that closes drive isles, closes building entries or closes public corridors shall be during low patient time and performed Sundays. f. The Contractor shall check in (in person) with the Administrative Officer of the Day (AOD) which is located in Patient Admitting for any work between 6:00 pm through midnight to 6:00 am or any time on weekends. Provide the name and cell number of the on-site supervisor to the AOD. At the completion of the work, check out with the AOD. 3. All building systems will be maintained in full operation at all times: -- 2 of 8 -- SOW 3/31/25 Page 3 of 8 a. Request system shutdowns in writing not less than 21 calendar days in advance. b. Shutdown work shall be during low patient time and performed between Saturday 6 pm through midnight concluding by 6 pm on the following Sunday. 4. Utilities. The contractor can use existing building utilities during the execution of this work. 5. Safety Assurance a. Compliance with Regulations. All work including the handling of hazardous materials or the disturbance or dismantling of structures containing hazardous materials shall comply with the applicable requirements of 29 CFR 1910/1926. Work involving the disturbance or dismantling of asbestos or asbestos-containing materials; the demolition of structures containing asbestos; and/or disposal and removal of asbestos, shall also comply with the requirement of 40 CFR, Part 61, Subpart A. All work shall comply with applicable federal, state and municipal safety and health requirements. Where there is a conflict between applicable regulations, the most stringent shall apply.
