CUI STATEMENT OF WORK CARPET REMOVAL AND INSTALLATION FOR ARMY SUPPORT ACTIVITY JBSA – FORT SAM HOUSTON, TX BLDG 145 JUNE 2026 1. DESCRIPTION OF SERVICE: The Army Support Activity requires removal of old carpet, and delivery and installation, of new commercial carpet tiling in Building 145 (2442 Stanley Road, Joint Base San Antonio – Fort Sam Houston, TX 78234) in all areas currently carpeted. Service will include the removal and replacement of furniture in the carpeted areas. 2. SCOPE: 2.1. The Contractor shall provide all flooring, equipment, components, and labor for the site-specific removal, installation and repair, as necessary, to support the installation of the new commercial carpet tile. All work performed by the Contractor shall be done in accordance with all applicable laws, regulations, and commercial practices. 2.2. The contractor is responsible for moving non-personal belongings/standard furniture in all spaces affected including cubicle furniture and replacing items after installation is complete. Items moved will be stored in the non-carpeted areas of the building while removal/installation occurs. Should additional storage space be required, it is the responsibility of the contractor. If either a Conex or box truck/trailer are required for storage the contractor shall submit a laydown plan for government approval. Personal belongings and breakable/pilferable/loose contents will be removed from work areas by government personnel. 2.3. The Contractor shall coordinate execution of installation with the Army Support Activity and shall ensure personnel are eligible to receive access to the installation and facility prior to the project. 2.4. WARRANTY: Contractor shall provide all commercial warranties for product as provided by the manufacturer. Warrantees shall be a minimum of one year on installation and workmanship and a minimum of five years on materials. 3. JUSTIFICATION: The Army Support Activity’s (B145) office carpet is worn and stained including visible indications of deterioration. Age of the carpet is estimated to be between 10-15 years, and life-cycle replacement is needed. -- 1 of 5 -- CUI 4. LOCATION: Project work is to be performed on Joint Base San Antonio-Fort Sam Houst. Project area is in Building 145, 2442 Stanley Road, JBSA-FSH, TX 78234 and includes all currently carpeted areas (approximately 5800 square feet). Attachment 1 includes a basic floor plan with carpeted areas highlighted. The building has sufficient parking and accessibility in the surrounding areas. The Contractor shall coordinate with the Facility Manager(s) to gain access to the facility. 5. INSTALLATION/MATERIALS: New carpet material will be Mohawk Group, SwitchCode, Umpteen (BT594) or like product and meet the below specifications. Contractor is also responsible for replacing associated cove base/baseboards with matching/coordinating material as approved by the government. 6. SAFETY. The Contractor shall provide all safety gear to include hard hats, eye protection, ear/sound protection, and any other safety equipment required for safe working conditions. The Contractor is also responsible for proper hazard signs and safety around the work area. 7. SECURITY. Personnel will be required to meet Air Force security requirements to receive entry on the installation. 7.1. Base Entry Requirements: The Contractor shall obtain base identification, and any required vehicle passes for all contractor personnel who make frequent visits to or perform work on the installation cited in the contract. The Contractor shall, within three (3) working days after receiving award, submit a base pass request to the customer for approval. The form shall include contract number, location, hours, date of birth, etc. The customer will endorse the request and submit it to Security Forces for processing. Submissions of late, inaccurate, or incomplete information could delay issuance of access credentials. During performance of the contract, the Contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials to the CO or COR for any employee who no longer requires access to the work site. Upon completion or termination of the contract or expiration of the identification passes, the primary Contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office. -- 2 of 5 -- CUI 7.2. Contractor Consent to Background Checks: The Contractor and, as applicable, subcontractor shall not employ persons for work on this contract if such employee is identified as a potential threat to the health, safety, security, general well- being or operational mission of the installation and its population. Nor shall the Contractor or subcontractor employ persons under this contract who have an outstanding criminal warrant as identified through the National Crime Information Center. Background checks will verify if a person is wanted by local, state, and federal agencies. All contractor and subcontractor personnel must consent to background checks. Contractor and subcontractor personnel who do not consent to a background check will be denied access to the installation. 7.3. Contractors shall ensure their employees and those of the subcontracts have the proper credentials allowing them to work in the United States. Persons later found to be undocumented, or illegal aliens will be remanded to the proper authorities. The contractor shall not be entitled to any compensation for delays or expenses associated with complying with the needs specified in this servie. 7.4. Contractor Registration of Vehicles on JBSA-Fort Sam Houston: All contractor or contractor employees' vehicles used for the performance of this contract shall comply with all local, state, and federal regulations.
