DPERFORMANCE WORK STATEMENT (PWS) Project Title: 108th ADA BDE/2-44 FCKY H2F Relocation Project Number: F-26-017 Location: Fort Campbell, KY PART 1 GENERAL INFORMATION 1. General Information. This is a non-personal services contract to provide 108th ADA BDE/2-44 FCKY H2F Relocation. The Government shall not exercise any supervision or control over the contract service providers performing the services herein. Such contract service providers shall be accountable solely to the contractor who, in turn, is responsible to the Government. 1.1. Description of Services/Introduction. The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items necessary to perform the contractor shall relocate the Holistic Health and Fitness (H2F) Area of Operations for the 2-44 Battalion FCKY. The primary objective is to disassemble, transport, and reinstall specialized flooring and equipment at the new facility while maintaining all existing manufacturer warranties and equipment integrity as defined in this Performance Work Statement (PWS). 1.2. Background. The H2F Area of Operation at 2-44 Battalion FCKY is required to move due to ARSTRUC changes. To retain the current flooring and equipment that was purchased and installed in 2023 while maintaining warranty and the specifications of the manufactures. This service will minimize the logistics and unit impact of the relocation of equipment. The relocation requires skilled labor and transportation of the equipment with specialized resources. 1.3. Objectives. The basic service objectives include the following: 1.3.1. The Contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to perform operations necessary for providing (Break down existing H2F equipment, removal of equipment, transport to new facility, re-installation of all equipment) at (2-44 Battalion FCKY). 1.3.2. Contractor will move flooring and equipment that was purchased and installed in 2023 while maintaining warranty and the specifications of the manufactures. This service will require minimal power equipment, no forklifts or trucks more than a ¾ ton to reduce contractors footprint. The relocation requires skilled labor and transportation of the equipment and contractor provided resources. 1.3.3. Clean up after Servicing: The Contractor shall remove all trash, debris, or surplus materials from the work site during and at the end of each workday and shall leave the work site clean of all debris when work is completed. 1.4. Place of Performance. The work under this contract is at Fort Campbell, KY. The current location is 7940 Destiny Blvd, Fort Campbell, KY 42223. All the equipment will be transported and reinstalled in location: 6871 A Shau Rd, Fort Campbell, KY 42223. The contractor shall have routine access to Government-controlled facilities. 1.5. Period of Performance (POP). Will be stated in the contract. 1.6. Recognized Federal Holidays. The contractor does not normally perform services on recognized federal holidays. When a holiday occurs on a Saturday, Federal employees normally observe the holiday on the previous Friday. When a holiday occurs on a Sunday, Federal employees normally observe the holiday on the following Monday. The contractor shall work on days Government personnel are working. 1.6.2. When an unforeseen installation closure occurs on a regularly scheduled day of work, the contractor will have the following options: 1.6.2.1. Reschedule the work so performance occurs the following day unless the following day falls on a weekend. 1.6.2.2. Reschedule the work on any day that is mutually satisfactory. 1.7. Hours of Operation. The contractor shall be responsible for providing services Monday through Friday between the hours of 7:30 AM – 4:30 PM Central Time (CT), except federal holidays or when the Government facility closes due to local or national emergencies, administrative closings, or other similar Government facility closings. 1.7.1. Performance of work by contractor personnel under the terms of this contract shall not interfere with regularly scheduled Government operational activities. 1.7.2. The exception to performance during non-duty hours is for issues of life, safety, or health concerns or otherwise at the direction of the Contracting Officer (KO). 1.8. Quality Control (QC). Quality Control is the responsibility of the contractor. 1.8.1. The contractor shall develop, implement, and maintain a written QC Plan (QCP). The QCP shall implement procedures and methodologies for monitoring and documenting contract performance to ensure compliance with all contract requirements. The QCP shall contain a systematic approach to monitor operations to ensure services comply with the standards defined in the PWS and Performance Requirements Summary (PRS). 1.9. Corrective Action Request (CAR). CARs manage contractual nonconformances. Nonconformance occurs when a contractor fails to meet contract terms, conditions, and/or specifications. Each time they make an observation of nonconformance, the Contracting Officer’s Representative (COR) will notify the contractor and ask the contractor to correct the problem or reperform the service. As applicable, the prime contractor shall address resolution of a nonconformance discovered at the subcontractor level. 1.10. Quality Assurance (QA).
