The Department of Veterans Affairs, RPO-East NCO 7 located in Tuscaloosa, Alabama anticipates the award of a Firm-Fixed price VISN (7) Building 143 Dietetics Flooring Renovation Repair/Replace at Tuscaloosa VA Medical Center Contract as identified in the attached SOW: Georgia, South Carolina and Alabama. This service will be acquired in accordance with FAR 12 – Simplified Procedures.
• The requirement will be a TOTAL SDVOSB SET-ASIDE.
• Statement of Work (SOW) information is attached for additional information.
The applicable North American Industry Classification System Code (NAICS) for this procurement is 238330: Other Electronic and Precision Equipment Repair and Maintenance. The Small Business Size Standards is $19 Million.

STATEMENT OF WORK (SOW)
Tuscaloosa VA Medical Center
PROJECT TITLE:
Building 143 Dietetics Flooring Renovation – Tuscaloosa VA Medical Center
PROJECT LOCATION:
Building 143 – Dietetics Department
PURPOSE:
The purpose of this project is to replace and upgrade the existing flooring system within the Dietetics Department by installing a Sherwin-Williams Poly-Crete SLB seamless cementitious urethane flooring system, including integral cove base, to improve durability, sanitation, slip resistance, and maintainability within food service areas.
SCOPE OF WORK:
The contractor shall provide all labor, supervision, materials, equipment, tools, transportation, and incidentals necessary to complete the following:
Pre-Construction Activities
Conduct site verification and field measurements.
Coordinate project phasing with Nutrition and Food Services (NFS) staff and Contracting Officer Representative (COR).
Develop an Infection Control and Dust Mitigation Plan.
Provide negative air machines, Zip Wall containment systems, and dust control measures throughout the project.
EQUIPMENT DISCONNECT/RECONNECT:
The contractor shall:
Disconnect, remove, and relocate all movable kitchen equipment necessary to complete flooring installation.
Coordinate shutdowns with facility staff.
Provide qualified personnel to disconnect and reconnect:
Commercial ovens
Steam kettles
Dishwashers
Prep tables
Refrigeration equipment
Ice machines
Food service carts
Sinks
Serving line equipment
Other kitchen equipment as identified during site verification
Protect all equipment from damage during relocation.
Reinstall and reconnect all equipment following flooring installation.
Verify proper operation of all equipment before final acceptance.
Repair or replace any equipment damaged during removal, relocation, or reinstallation.
FLOORING INSTALLATION:
The contractor shall:
Remove existing floor coatings as necessary.
Prepare concrete substrate by grinding/scarifying to achieve CSP 3–4 profile.
Perform moisture testing.
Install Sherwin-Williams Poly-Crete SLB flooring system.
Install 6-inch integral cove base.
Install flooring in accordance with manufacturer specifications.
Follow approved phasing plan to maintain dietary operations.
QUALITY CONTROL:
The contractor shall:
Conduct daily inspections.
Ensure compliance with manufacturer installation requirements.
Maintain clean and safe work areas.
Perform final punch-list corrections.
FINAL ACCEPTANCE:
The contractor shall provide:
Manufacturer warranty documentation.
Product data sheets.
Maintenance recommendations.
Final inspection and operational verification of all reinstalled kitchen equipment.

SAFETY:
Contractor provides proof that all employees have tested negative for Tuberculosis prior to arriving on campus. Contractor to provide proof that the Supervisor has completed the OSHA 30 course, and proof that all employees have completed the OSHA 10 course prior to arriving on campus.
IDENTIFICATION, PARKING, SMOKING, AND VA REGULATIONS:
The Contractor's FSE shall wear visible identification always while on the premises of the Tuscaloosa VA Medical Center. Contractor shall only park in the appropriate designated parking areas.
