Performance Work Statement: Carpet Installation U.S. Consulate Johannesburg 1 Sandton Drive, Johannesburg, South Africa 1. Purpose This contract covers the removal of existing carpets and installation of new USG-supplied carpets throughout the Consulate office building, including professional furniture moving services. 2. Scope of Work The Contractor shall: Remove and dispose of existing carpets (off-site removal weekly via 6m³ skip or equivalent) Prepare floors (clean and dust-free) prior to installation. Install new carpets per manufacturer specifications. Parquet layout. Move and reinstate all office furniture. Maintain clean work areas daily. USG-Supplied Materials: Area Product Adhesive Consular Section Bentley Mimitsu San NexStep Cushion 18"x36" Tile (6AHS20AA0T) Healthbond Ultra Green 2399 (82312); 1/16"x1/32"x1/32" U-notch trowel Remainder of Building Milliken Linen 2.0, SIN 314110, Color Crommelin, 1m tile Milliken Non-Reactive Carpet Adhesive Refer to attached Bentley and Milliken installation guidelines. The Contractor shall re-measure all areas prior to commencing work. Estimated floor Areas: Floor Area First Floor (incl. Consular Section – 466 sqm) 1,195 sqm Second Floor (incl. CAA escorted area – 273 sqm) 1,373 sqm Total 2,568 sqm 3. Access & Working Hours Standard hours: 07:00–16:00, Monday–Friday (excluding public and U.S. holidays). Consular Section: Accessible Fridays from 12:00 and weekends only. Weekend work is required; deviations require 24-hour advance notice to the COR. All Contractor personnel will be escorted; security clearances arranged by the Consulate. 4. Supervision & Quality Control The Contractor shall provide a full-time, competent on-site supervisor responsible for: Regular quality inspections to ensure compliance with manufacturer standards. Adherence to all Health & Safety regulations, including noise and dust control. Ensuring uninterrupted access to the building. Submitting an installation program for COR approval prior to commencement. 5. Personnel & Conduct The Contractor shall maintain discipline on-site and take all reasonable precautions to prevent unlawful or disorderly conduct. The Government reserves the right to direct removal of any employee who fails to meet conduct standards; the Contractor shall replace such personnel at no additional cost. 6. Insurance Requirements The Contractor shall provide evidence of the following insurance within 10 days of contract award. The policy shall name "the United States of America, acting by and through the Department of State" as an additional insured. General Liability (ZAR): Coverage Per Occurrence Cumulative Bodily Injury R250,000 R1,000,000 Property Damage R300,000 R1,000,000 Additional insurance required by local law or customary practice must also be obtained. The Contractor holds the Government harmless from all claims except those arising from gross negligence on the part of the Government. 7. Safety Requirements The Contractor shall designate a competent safety person on-site and comply with all U.S. Embassy and applicable local/OSHA safety standards, including: Pre-work: Site hazard review, safety briefing, and submission of safety file (including proof of employee insurance) PPE: Required for all personnel; spare PPE maintained on-site Barricading: Barricades and warning signs maintained throughout the project Chemicals: Safety Data Sheets (SDS) for all chemicals submitted to COR/POSHO Emergency: Fire extinguisher and first aid kit on-site at all times; emergency response instructions followed Electrical: All work performed de-energized; conductive jewelry prohibited during electrical tasks. Fall Protection: OSHA standards apply; fall protection training certificates submitted to POSHO via COR at least 8 working days before work begins High-Risk Work: Training certificates and work permits submitted to COR at least 3 days prior; minimum two employees present for all high-risk tasks Prohibited: Smoking (except designated areas), alcohol, marijuana, and recreational substances on USG property. 8. Contractor Selection Criteria Demonstrated history and track record as a flooring installation company. FITA accreditation (carpet installation) will receive preference. AMOSA accreditation (furniture moving) will receive preference. Three contactable references for comparable projects (similar scope and size)
