1. Title
Western Oregon Service Unit Laboratory Supplies
2. Background
The Chemawa Indian Health Center Laboratory Department performs laboratory testing necessary to support medical providers in the diagnosis and treatment of diseases. The Western Oregon Service Unit requires a reliable source of laboratory supplies to maintain uninterrupted laboratory operations and support accurate and timely specimen collection, testing, diagnosis, and treatment activities.
3. Scope
The contractor shall provide laboratory supplies listed in Attachment 1 - Schedule of Items on an as-needed basis upon order from the Laboratory Medical Technologist or other authorized Government personnel. The contractor shall receive orders, coordinate shipment as needed, and deliver supplies to the designated facility within the required delivery timeframe.
4. Supply List and Product Requirements
The contractor shall provide all laboratory supplies listed in Attachment 1. The supplies include items used for Chemistry Testing, Hematology Testing, Molecular Testing, Microbiology Testing, COVID-19/Antigen Testing, Urinalysis, Point of Care Testing, Coagulation Testing, Phlebotomy, and Miscellaneous laboratory functions. Example manufacturer and part/reference numbers are provided for identification of the required product type and expected form, fit, function, quality, and packaging. Equivalent items may be proposed, but must meet or exceed the referenced item characteristics and must be identified in the pricing schedule.
5. All-or-None Requirement
The vendor is required to bid on all items. A quotation that does not include pricing for all listed items for the base period and each option period may be found unacceptable.
6. Ordering
Supplies will be ordered on an as-needed basis by authorized Government personnel, including the Laboratory Medical Technologist. The contractor shall accept orders by email or another mutually agreed ordering method and shall confirm receipt of each order.
7. Delivery
The contractor shall deliver supplies within 10 business days after order, unless otherwise approved by the Government. Deliveries shall be made F.O.B. Destination during normal business hours, 7:30 AM to 4:30 PM local time, Monday through Friday, excluding Federal holidays.
8. Delivery Address

9. Backorders, Discontinued Items, and Substitutions
The contractor shall notify the Government in writing within two (2) business days after the contractor becomes aware, or reasonably should have become aware, that an ordered item is on backorder, on indeterminate backorder, unavailable, or discontinued. The notice shall identify the affected item, order number, reason for nonavailability, estimated availability date if known, and any proposed comparable alternative. If an item is unavailable, the contractor shall propose a comparable alternative. No substitution shall be shipped unless accepted in writing by the Government.
10. Product Condition, Shelf Life, and Packaging
All supplies shall be new, unused, undamaged, and suitable for clinical laboratory use. Products shall not be expired at delivery. Sterile, reagent, control, diagnostic, specimen collection, personal protective equipment, and infection-control products shall maintain packaging integrity and include lot number and expiration information when applicable. Products shall be packaged, stored, and shipped in accordance with manufacturer requirements to prevent damage, contamination, product deterioration, or loss of required storage conditions during shipment.
11. Infection Control, Safety, and Security
The contractor shall comply with Chemawa Indian Health Center infection control, safety, and security procedures, practices, and standards when delivering supplies or otherwise accessing the facility. Delivery personnel shall follow facility check-in and access directions.
12. Government-Furnished Information and Equipment
The Government will provide Attachment 1 - Schedule of Items and ordering information. No Government-furnished equipment will be provided.
13. Performance Monitoring
The Government will monitor performance through verification that ordered items are received, inspection for order completeness and accuracy, confirmation that supplies are not damaged or expired, confirmation that supplies meet applicable safety and usability standards, review of delivery timeliness, and confirmation that substitutions or equivalents have been accepted before shipment.
14. Inspection and Acceptance Criteria
Supplies will be accepted upon verification that items match the order, quantities are correct, packaging is intact, items are undamaged and not expired, manufacturer-required storage and handling conditions have been maintained when applicable, and any accepted equivalent items meet the Government requirement. Incomplete, damaged, expired, incorrect, or nonconforming supplies may be rejected and shall be replaced at no additional cost to the Government.
15. Period of Performance
Base: 07/09/2026 - 07/08/2027; Option Year 1: 07/09/2027 - 07/08/2028; Option Year 2: 07/09/2028 - 07/08/2029; Option Year 3: 07/09/2029 - 07/08/2030; Option Year 4: 07/09/2030 - 07/08/2031.
16. Invoicing
Invoices shall be submitted through the Department of Treasury Invoice Processing Platform (IPP) after delivery and Government acceptance. Invoices shall identify the applicable order, CLIN(s), item descriptions, quantities, unit prices, extended prices, delivery date, and supporting packing slip or receiving documentation.
