Page 1 of 2 Performance Work Statement NAVSPECWARCENTER Rock Wall Inspection, Maintenance and Repair Services 1. BACKGROUND 1.1 The rock wall located at NSWCEN Medical requires inspection, maintenance and repair services in order to ensure safe operations. 2. OBJECTIVES 2.1 Establish a five-year Indefinite Delivery Indefinite Quantity (IDIQ) contract for inspection, maintenance, and repair services for the NSWCEN Medical rock wall in order to ensure the saftey of the patients that utilize it for Rehabilitation. 3. SCOPE 3.1 The contractor shall provide all personnel, tools, parts, supervision and other items necessary to perform inspection, maintenance and repair services to include inspecting and testing of the equipment to ensure safe and proper functionality. Inspections are required annually and will be issued via individual task orders. Repair services are required on an as-needed basis and will be issued via individual task orders. 4. REQUIREMENTS 4.1 Requirements 4.1.1 Annual Rock Wall Inspection 4.1.1.1 The contractor shall conduct an annual inspection of the climbing walls primary structure, substrates, surface, and hand hold anchors to determine their condition. As part of the annual inspection, the contractor shall strip and wash hold anchors and reset climbing routes. 4.1.1.1.1 Contractor shall provide a written report detailing the structural evaluation and provide a list of required and recommended repairs NLT than 10 days after completion of service to the Government TPOC. The report shall also include quotes for required and recommended repairs to ensure safe operations. 4.1.2 Repairs (As Required): 4.1.2.1 Required and recommended repairs identified as part of the annual inspection shall be negotiated at the task order level and issued via individual task orders. 4.1.3 Other Services To Be Negotiated 4.1.3.1 Other Rock Wall services may be included as required on a to-be-negotiated basis. 5. CONTRACTOR TRAVEL 6.1 Any travel associated with performance of these services shall be included within the Firm Fixed Price of the service. 7. SAFETY 7.1 Contractor Responsibility -- 1 of 2 -- Page 2 of 2 7.1.1 Nothing in the Statement of Work, nor any Government action or failure to act in performance of the contract shall relieve the contractor of its responsibility for the safety of the contractor's personnel and property, the Government's personnel and property, and the general public. Furthermore, nothing in this PWS shall relieve the contractor of its responsibility for complying with applicable Federal, State, and Local laws, ordinances, codes, and regulations (including those requiring applicable licenses and permits) in connection with the performance of this task order. 7.1.2 The Government process must be in place that will ensure any mishaps are reported, investigated, and recorded. This information will be input into the Web Enabled Safety System (WESS) by the Government. 7.1.3 Safety related problems or mishaps involving or observed by the contractor shall be addressed immediately to the designated TPOC. A formal written report shall be prepared and submitted to the Safety Officer within 2 working days of the incident. 8. Disclosure of Information. 8.1 Information made available to the contractor by the government for the performance or administration of this effort shall be used only for those purposes and shall not be used in any other way without the written agreement of the Contracting Officer. The contractor agrees to assume responsibility for protecting the confidentiality of government records, which are not public information. Each contractor or employee of the contractor to whom information may be made available or disclosed shall be notified in writing by the contractor that such information may be disclosed only for a purpose and to the extent authorized herein. 9. GOVERNMENT FURNISHED PROPERTY/EQUIPMENT/INFORMATION (GFP/GFE/GFI) 9.1 N/A 10. QUALITY CONTROL 10.1 The contractor shall implement and maintain a Quality Control Plan (QCP) to ensure work performed conforms to the scope of work and meets the requirements under this PWS. The QCP shall, at a minimum provide a method for performing inspections; identifying, correcting and preventing problems/defective service; addressing customer complaints, and improving the quality of services over the life of the contract. 11. QUALITY ASSURANCE 11.1 The Government reserves the right to perform inspections and surveillance to evaluate the Contractor’s compliance to the contract terms and performance of the requirements in the PWS. The Government will assign TPOC who will be responsible for monitoring performance in accordance with the requirements of the PWS. 11.2 Contract Discrepancy Report (CDR). In the event of unsatisfactory contractor performance, the CO will issue a CDR that will explain the circumstances and findings concerning the incomplete or unsatisfactory service. The contractor shall acknowledge receipt of the CDR and respond in writing as to how he/she shall correct the unacceptable performance and avoid a recurrence. The Government will review the contractor's corrective action response to determine acceptability and will use any completed CDR as part of an overall evaluation of Contractor performance when determining present or future contractual actions. 12. PLACE OF PERFORMANCE 12.1 Place of performance is NSWCEN Medical Coronado, CA. 12.2 Additional locations may be added as necessary and will be incorporated via contract modification. 13.
