Statement of Work (SOW) For HAAS VF-5/40 and TM-2 Machine Repair 354 MXS/MXMFM, Eielson AFB, AK July 2026 -- 1 of 3 -- HAAS VF-5/40 Machine Repair 1. Background & Scope The 354th Maintenance Squadron (354 MXS) at Eielson AFB intends to award a sole-source firm-fixed-price contract to Phillips Corporation, Federal Division, for the repair of two (2) Haas CNC 3-axis machines (VF-5 and TM-2 models) located at Eielson AFB, Alaska. 2. Equipment Identification The contractor shall perform maintenance on the following government-owned equipment: Manufacturer Model Serial Number (S/N) Haas Haas VF-5/40 TM2 1081702 1084328 3. Specific Tasks & Deliverables The contractor shall execute the following repair actions: Task Area Requirement Description Hardware Replacement Furnish and install two (2) Service Kits, Battery Boards (Part #: 93- 32-4307). System Initialization Reinitialize the machine software and systems following the battery board installation. Functionality Testing Conduct comprehensive testing of machine functions to ensure the equipment operates to Original Equipment Manufacturer (OEM) specifications. Debris & Cleanup Remove any generated waste or packaging from the installation site upon completion. 4. Period and Place of Performance All work must be coordinated with the 354 MXS point of contact prior to arrival. Warranty & Acceptance The government requires standard commercial warranties for the installed parts and labor. Acceptance of services will occur upon successful completion of the functionality testing and sign-off by the 354 MXS POC. 6. Documentation The contractor shall provide physical copies of any and all functionality testing results to the 354 MXS POC within 10 days of acceptance of services. -- 3 of 3 --
