Attachment 4A 1 STATEMENT OF WORK BLDG #565 Carpet Tile 1st Floor 16 Dec. 2025 1. SCOPE OF WORK The work will include providing all necessary labor, equipment, transportation, materials, and support accessories, to remove existing glue down carpet, wall base, stair nosing and transitions. Contractor will be responsible for lifting or moving any furniture required for proper removal and installation of carpet. Furnish and install selected 24”x24” carpet tiles with “Tile Tabs” adhesive tabs, 4 inch rubber base and transitions to match existing. Stair nosing, riser and treads will match existing in color and quality. Contractor may be required to man the job to support an elevated work schedule for completion 1 month from start date. For the auditorium area, if approved under this contract, stair nosing, riser and treads will match existing in color and quality. 2. DEMOLITION The contractor shall demo existing flooring material, all transition and all wall base and stair nosing in designated areas. All building material shall be disposed of off MacDill AFB. 3. FLOOR PREPARATION Anything other than mechanical removal of flooring material and non-water base adhesives and any additional floor preparation that may be required as a result of unforeseen conditions shall be brought to the attention to the Contracting Officer Representative (COR). 4. ASBESTOS ABATEMENT Should the Contractor encounter Asbestos Containing Material (ACM) which must be disturbed, the Contractor shall cease all work that would disturb the ACM and shall immediately notify the COR. 5. SITE MAINTENANCE AND CLEANUP a. SITE MAINTENANCE – The Contractor shall protect adjacent property, building, and their contents from dust, dirt, or other materials. b. CLEANUP – Daily, the Contractor shall collect all trash, debris, refuse, garbage, etc., which is generated and place it in the appropriate containers with lids or approved covers. 6. LOCATION OF WORK Work is located at building 565 1st floor 7. PERIOD OF PERFORMANCE All work is to be started within 30 days and completed within 60 days of purchase order award.
