Minot AFB Building 864 JLG Boom Lift Repair Jan 6, 2026, Page 1 STATEMENT OF WORK – SOW FOR REPAIR OF PHASE SECTION JLG BOOM LIFT AT MINOT AFB, ND -- 1 of 3 -- Minot AFB Building 864 JLG Boom Lift Repair Jan 6, 2026, Page 2 1.0 INTRODUCTION. This Statement of Work (SOW) defines the requirements for the contractor to inspect, repair, and replace all worn and damaged parts, replace all 4 tires with foam filled tires, and replace hydraulic and engine oil and filters on the JLG M600JP Boom Lift (Equipment #COE92875) utilized by Dock 8’s Maintenance flight at Minot AFB, ND. 2.0. SCOPE. The contractor will provide all materials, labor, management, and other incidentals necessary to meet the requirements as stated in this SOW. All materials provided must be new and not refurbished. 2.1. Inspection. The contractor will perform an inspection and recertification in accordance with ANSI/SAIA A92.22 as well as manufacturer’s specifications and applicable manufacturer's bulletins (see Appendix A). Inspection must be conducted by a qualified Mobile Elevating Work Platform (MEWP) service technician. The contractor will provide a list and estimates to correct any deficiencies or repairs required to bring the lift up to passing ANSI/manufacturer standards. Upon successful completion of the test, the contractor shall provide the Government with a formal written certification stating the lift has passed the annual inspection in accordance with ANSI/SAIA A92.22 and manufacturer standards. The contractor shall update the inspection identification means on the lift. This marking must clearly identify the date this annual inspection was performed and the interval at which future annual inspections are required. 2.2. Replacement. The contractor will complete all items for the JLG M600JP Boom Lift (Equipment #COE92875) located at MINOT AFB including: replacement of a leaking axle lockout cylinder, repairing and resealing a leaking wheel hub, and replacing a leaking drive motor hose. Additionally, the contractor must dismount and replace four weather-checked tires with Blackstone OTR 36 x 14LL - 22.5 or a similar substitute that are foam filled. Furthermore, the service provider will conduct standard preventative maintenance consisting of a full engine oil change and the replacement of the engine oil, fuel, and hydraulic filters. All salvageable or unused parts identified during the inspection will be transferred to the government vehicle custodian. 2.3. Transportation. All work shall be performed at the contractor’s facility. The Government assumes full responsibility for transporting the equipment to the contractor's site and retrieving it upon completion of the service, provided the contractor's repair facility is located within a 120- mile radius of the dispatching installation. Contractor locations exceeding this 120-mile limit will not be eligible for Government-provided transport. To ensure smooth logistics, the contractor must coordinate the specific drop-off and pick-up dates with the designated Government Point of Contact (POC) at least seven (7) days prior to the commencement of work. 3.0. GENERAL REQUIREMENTS. 3.1. Waste Disposal. The contractor will dispose of waste to include unserviceable parts from this project off base and IAW Federal, State, local, and base environmental laws, and regulations. 3.2. Fire Suppression. Any new wiring or hosing shall use adequate fire blocking to meet or exceed the current standard in place. 3.3. Installation Access.
